Design notes
The portrait version redraws the bedside care board for a tall page instead of shrinking the wide one. The name plate takes most of the board's width under the MY NURSE bar, so the name is the largest thing on the sheet. Below it, a two-column grid holds the note of thanks beside the program, the date, credential ID and nominator beside the three things the nurse gave, and a full CARE TEAM row with two signatures and the QR. A portrait shape fits a frame on a unit wall or a hospital notice board.
About
Nurse Appreciation Certificate, Care Board Portrait
A nurse who is thanked by name often keeps that thanks: on the break room wall, in a portfolio for a clinical ladder review, or beside a nurse of the year plaque. Nurse managers, chief nursing officers and patient experience teams give a certificate of appreciation for Nurses Week, after a patient or family nomination, or when a peer award picks one nurse from the unit for compassion, skill and support for the team.
Wording on the template“Thank you for the care you give on every shift: the skill, the patience and the kindness that patients and families remember long after they go home. Presented with gratitude and respect, on behalf of everyone you cared for and everyone you worked beside.”More wording ideas for nurse appreciation certificates

Subject: name, a thank-you from your patients and your care team
Hi name,
This Nurses Week we want to say what we do not say often enough: thank you. Patients and families leave our unit remembering the nurse who explained, listened and stayed calm when it mattered, and very often that nurse was you.
Your certificate of appreciation is ready. Open it to read the message, download it to print for your locker or the break room, and share it on LinkedIn if you like.
With thanks from your whole care team.

Post for the recipient to share
Nursing is mostly small moments nobody writes down, so it meant a lot to receive a certificate of appreciation from my unit this Nurses Week, with a nomination from a patient's family. Thank you to the nurses, aides and doctors I share every shift with. We do this together. #NursesWeek #Nursing

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
