Design notes
This landscape design sets the record as a journal figure. Under the name and the activity in italic, a full-width forest plot lists the topics covered, each with its credits, and a burnt-sienna diamond marks the total credits claimed. A bracketed line holds space for the provider's own credit designation statement, followed by a short summary, the learning format, duration and date of issue, and a signature. The record number sits beside a verification QR code at the foot. Navy serif type on ivory paper gives it the calm of a medical journal; it suits live courses and conferences.
About
CME Certificate Template, Forest Plot Landscape
A CME certificate is the record a physician keeps after a continuing medical education activity: a conference session, a grand rounds series, a journal-based module or an online course. Accredited providers, hospitals, universities and specialty societies issue it with the activity title, the date and the credits claimed, and clinicians file it for licence renewal, hospital privileges and maintenance of certification audits.
Wording on the template“The physician named above participated in this continuing medical education activity and completed its learning assessment, bringing current evidence and updated guidelines into clinical practice.”More wording ideas for continuing medical education (cme) certificates

Subject: Your CME certificate is ready, name
Hi name,
Thank you for taking part in our continuing medical education activity and for completing the evaluation. Your certificate of credit is ready. It shows the activity, the date and the credits you claimed, with a credential ID that a licensing board or your employer can verify.
Open it to view the record, download a copy for your CME file or print it, and share the badge on your professional profile if you wish.
With thanks from the education team.

Post for the recipient to share
I completed a continuing medical education activity this week and claimed my credits for the current cycle. Keeping up with new guidelines and diagnostic advances is part of caring for patients well, and the sessions gave me ideas I am already using in clinic. Thanks to the faculty for a well-run program. #CME #MedicalEducation

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it on its side, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
