A home health aide certificate template gives a training provider a clean way to record a finished course: the classroom and supervised practical hours, the skills the trainee practised and the date the training ended. Home health agencies that train their own staff, career schools, community colleges and adult education programs hand it out at the end of the course, often before the new aide starts visiting clients alone.
It is a record of training from the provider, not a state certification. A good certificate for home health aide training names the provider and the program, lists the main skills (personal care, vital signs, home safety, infection control, reporting changes), shows the hours and carries the signature of the program director or the supervising nurse. If your program sets a refresher date, add a valid-until date.
The designs here come as printable certificates in A4 and US Letter and as digital badges to share online.

FAQ
Questions, answered
Yes, many agencies run their own home health aide certificate course, and so do career schools and community colleges. Federal rules for Medicare-certified agencies let an agency train its own aides, except for a period after certain survey findings or penalties, and some states add their own approval for training programs. The certificate names whoever gave the training.
It is the hands-on part of the course: the trainee does real care tasks on a person while a registered nurse, or a licensed practical nurse working under an RN, watches. For aides at Medicare-certified agencies, federal rules ask for at least 16 hours of it within a course of at least 75 hours. Showing the practical hours apart from the classroom hours makes the record clearer.
Not always. A course certificate shows that the training was finished; a competency evaluation checks each skill, with the hands-on tasks watched by a nurse. If your program includes the evaluation and the trainee passed it, say so on the certificate with the date. If a state test or registry is involved, that comes from the state, not from the training provider.
The skills overlap, but a home health aide works alone in the client's home, so the course stresses home safety, following the care plan and reporting changes to the nurse by phone or in notes. A nursing assistant course centres on care in nursing homes and hospitals. Title the certificate for the program the person really finished.
Often, but the new employer decides and usually checks the aide's skills before the first visits. Under federal rules, an aide who has not worked for pay as an aide for 24 months in a row must train or be evaluated again. A credential ID and a QR code let the employer confirm the record with you in seconds.
