Design notes

This landscape design draws the aide's working day as a navy route on a warm street map. Five numbered visits, each with a clock time, carry the skills covered, and the route ends at the QR code for verification. Above the map sit the name, the program, a short statement and the hours; below it are two signatures, the issue and valid-until dates and the credential ID. It suits agency training programs and home care courses.

About

Home Health Aide Certificate Template, Visit Route Landscape

Home health aides look after clients in their own homes. They help with bathing, dressing and meals, check vital signs, keep the home safe and tell the supervising nurse when something changes. Home health agencies, career schools and community colleges give a certificate of completion when a trainee finishes the classroom hours and the supervised practical training, often before the aide starts visits alone. It is the training provider's record of the course, not a state certification.

Wording on the template“Completed classroom and supervised practical training in caring for clients in their own homes: following the care plan, keeping each visit safe and reporting changes to the supervising nurse.”

Printing

Printing it well

Print at actual size

Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.

Use card, not paper

A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.

Frame it

A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it on its side, as designed.

Mind the edges

Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
