An aerial lift certificate template gives you a clear record for each person you train on mobile elevating work platforms (MEWPs), such as scissor lifts and boom lifts. Equipment rental training centres, employers' safety teams and trade schools hand one out when a worker has finished the course and passed a practical evaluation at the controls. The designs here come as certificates (A4 and US Letter) and as badges.
A good one names the operator and the machines they were evaluated on, lists what was covered, such as the pre-use inspection, fall protection and tip-over hazards, and shows the date, the standard followed, who signed and when a refresher is due. It is the training provider's record of the course, not a licence: the employer still decides who may operate a lift on site.
Wording
What to write on a aerial lift operator training certificate
Use these lines as they are, or as a start for your own. Every template comes with wording you can change in the studio.
Wording for a scissor lift training certificate
- “Has completed aerial lift operator training and passed the practical evaluation on a scissor lift.”
- “Has shown a full pre-use inspection, a check of the work area and safe travel with the platform raised.”
- “Was evaluated at the controls of scissor and boom lifts in a hands-on session.”
Aerial lift refresher training wording
- “Has completed refresher training and a new practical evaluation on mobile elevating work platforms.”
- “Has completed refresher training before moving to a boom lift.”
- “This training record is valid until the date shown; a refresher is due before then.”
Short lines for the badge or the email

FAQ
Questions, answered
The operator's name, the types of lift they were evaluated on, the topics covered, the date of the practical evaluation, the standard the course followed and the signature of the person who ran the evaluation. A credential ID and a valid-until date make it easy to check later.
Not always. A scissor lift rises straight up over its base, while a boom lift reaches out past it, uses different controls and needs a harness and lanyard in the basket. The certificate should name each machine the person was evaluated on.
No. It is the training provider's record that the person completed the course and the practical evaluation. In the United States there is no national aerial lift licence; the employer makes sure that only trained and authorised people operate a lift. It does not replace an operator card from any scheme.
Many employers and training providers set a refresher every few years and show it as a valid-until date. Retraining is also wise sooner: after an accident or a near miss, when an operator is seen using a lift unsafely, or before they move to a type of lift they have not used.
The trainer or practical evaluator who watched the operator at the controls, and often a training manager or the employer's safety lead as a second signature.
A scissor lift seen from the side: two crossed chains of arms joined by pins, fixed at one end and rolling at the other, under a deck with a guardrail. The name stands on the deck, and the pins lead to the parts of the record.
