Design notes

On this tall page the goniometer fills the upper half: its scale frames the title and the name, and its arms lie at full extension under them, with the credential ID and issue date at the arm ends. Below, one centred column holds the program and a statement, then a straight 0 to 180 strip where each skill is marked as a reading: manual therapy, therapeutic exercise, clinical reasoning and outcome measures. Duration and credits follow, then two signatures and a QR code. A portrait suits a binder, a frame on the clinic wall or a printed portfolio.

About

Physical Therapy Certificate Template, Full Range Portrait

Students on clinical placement, newly qualified clinicians and experienced physical therapists all keep a record of the courses they finish, for appraisals, job applications and their own learning log. Universities, rehabilitation centres and continuing education providers issue a certificate of completion once the course hours and the practical assessment are done, whether the topic is spinal mobilisation, post-operative rehab or running gait.

Wording on the template“Awarded for completing this physical therapy course: applying manual therapy and therapeutic exercise with sound clinical reasoning, and measuring each patient’s progress, session by session, with validated outcome measures, from the first assessment to a full return of movement.”

Subject: name, your physical therapy course certificate is ready

Hi name,

Congratulations on completing the course. Your certificate of completion is ready: it records the course title, the hours, the skills you practised and the signatures of the course team.

You can download it for your portfolio or appraisal file, print it, and share it on your professional profile. Anyone who scans the QR code or opens the link can check that it is genuine.

Thank you for the effort you put into the practical sessions. We hope to see you on a future course.

Best wishes, The course team

Hyperstack fills in each person’s name and link when it sends.

Post for the recipient to share

I have just completed a course in manual therapy and therapeutic exercise, with hands-on practice in assessing movement and measuring progress with outcome measures. Thank you to the tutors and to everyone I practised with. Looking forward to bringing these skills to my patients. #Physiotherapy #PhysicalTherapy #Rehabilitation

Printing

Printing it well

Print at actual size

Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.

Use card, not paper

A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.

Frame it

A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.

Mind the edges

Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
