Design notes

Here the chart stands upright. The side tab runs down the long right edge with five coloured labels that carry only their codes, PI, VS, IN, PH and MR, and the ISSUED band at the foot wraps the date onto two lines. The face sheet explains the codes in a small key titled skills on this chart, under the name, the program and the statement. Field rows for the credential ID, duration and externship, two signatures and a QR code finish the page. It is a good choice for programs that print certificates for graduation folders.

About

Medical Assistant Certificate, Chart Folder Portrait

Graduates of a medical assistant course often frame their completion record or keep it in a portfolio for clinic interviews, and a tall page fits a file, a wall or a printed packet better than a wide one. Training providers issue it when the learner has passed the classroom modules, the lab skills checks and the hours of supervised externship in a physician's office or outpatient clinic.

Wording on the template“Has completed the training of a medical assistant, including a supervised clinical externship, and has shown the competence to work at the front desk and in the exam room: scheduling, rooming patients, taking vital signs and keeping accurate records.”

Subject: name, your medical assistant program certificate is here

Hi name,

Well done. You have completed the medical assistant program: the classes, the lab practice and your externship in the clinic. You learned to run the front desk and to work beside the provider in the exam room, and your supervisors saw it.

Your certificate of completion is ready at the link below. Download it, print a copy for your job applications and share it on LinkedIn or with clinics you are talking to.

We wish you a great start in your new role.

Hyperstack fills in each person’s name and link when it sends.

Post for the recipient to share

I have completed my medical assistant program, including the externship at a busy family practice. I learned to check patients in, take vital signs and keep the chart accurate, and I learned a lot from the team who supervised me. Thank you to my instructors and classmates. Now I am looking for my first role in a clinic. #MedicalAssistant #AlliedHealth #Healthcare

Printing

Printing it well

Print at actual size

Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.

Use card, not paper

A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.

Frame it

A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.

Mind the edges

Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
