Design notes
On the landscape page the five tiers of the hierarchy of controls fill the left side, and their edges run across the sheet as fine rules that divide the record into five rows: the title, the name, the program and statement, the topics with duration, track and standard, and the signature with the issue date, valid-until date, credential ID and QR code. A technical, manual-like look in navy on cream, suited to a site office wall or a training folder.
About
OSHA Training Certificate Template, Hierarchy of Controls
Outreach-style safety courses teach workers to recognise hazards on the job and to choose how to control them, from walking-working surfaces and electrical safety to PPE and their rights at work. Safety training firms, college workforce programmes and in-house EHS departments run them as 10-hour or 30-hour courses for general industry or construction, and many hand out their own completion certificate on the last day. That certificate is the provider's record of the course. The official course card of the Outreach Training Program is a separate document, and this template does not stand in for it.
Wording on the template“Completed a workplace safety training course in hazard recognition, avoidance and control, and can identify common hazards on the job and apply the hierarchy of controls to reduce risk.”More wording ideas for osha safety training certificates

Subject: Your safety training certificate, name
Hi name,
Thank you for taking part in our workplace safety training. Your certificate of completion is ready.
It lists the hours, the topics we covered and the standard the course followed. You can download it, print a copy for your site folder, or share it on LinkedIn. A supervisor or a client can check it at any time with the QR code.
Keep an eye on the valid-until date so you can book a refresher in time. Stay safe out there.
Hyperstack fills in each person’s name and link when it sends.
Post for the recipient to share
I have completed a 10-hour workplace safety course on OSHA standards for general industry. We worked through hazard recognition, electrical safety, walking-working surfaces and the hierarchy of controls, and one lesson stays with me: remove the hazard first, and treat PPE as the last line of defence. Thanks to our trainer for real examples from the floor. #WorkplaceSafety #SafetyFirst #HazardControl

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it on its side, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
Certificate or badge?
Send both. They do different jobs.
