Design notes
This tall version reads down the centre line like a descent. The top of the shaft opens under the ground line with the first test point in its cone; the title, name, program and statement follow; a short length of hose with the middle point divides them from the record grid of skills, details, dates, credential ID and QR code. At the foot, the floor of the space and the bottom probe sit between the instructor's and the safety manager's signatures.
About
Confined Space Certificate Template, Manhole Portrait
Before a crew lowers anyone into a sewer chamber, a storage tank or a utility vault, the entry supervisor checks that every person on the permit has been trained for their part. Entrants learn the hazards and how to get out, attendants learn to stay outside, keep count and call for rescue, and supervisors learn to test, ventilate, isolate and cancel a permit when conditions change. A portrait certificate fits the personnel file, a training binder or the wall of a depot.
Wording on the template“Trained to test the atmosphere before and during entry, to work under an entry permit as entrant, attendant or entry supervisor, and to plan ventilation, isolation and non-entry rescue.”More wording ideas for confined space training certificates

Subject: name, your confined space training certificate is ready
Hi name,
Thank you for completing confined space entry training. You now know how to test the air before and during entry, how the permit works, what the entrant, the attendant and the supervisor each do, and how a rescue is planned from outside.
Your certificate is ready. Open it to see it, download a copy to print for your training file, or share it on LinkedIn. It shows when your refresher is due, and we will remind you before then.
Thank you for helping everyone come back out safely.
Hyperstack fills in each person’s name and link when it sends.
Post for the recipient to share
I completed confined space entry training this week. We practised testing the air at every level before entry, working under an entry permit, and the jobs of the entrant, the attendant and the supervisor. The biggest lesson: a rescue is planned before anyone goes in. #ConfinedSpace #WorkplaceSafety

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
