Design notes
The page runs down one centred axis. The logo and organisation sit at the top, then a small stability triangle whose front axle line is drawn out to both margins as the header rule. Below come the title, the operator's name in large condensed capitals, the program and a statement. A riveted plate holds the assessed skills on the left and the record on the right, including the standard, truck class and valid until date. Two signatures flank the QR code.
About
Forklift Training Certificate Portrait, Stability Triangle
Forklift certificates are often framed in a break room or kept in an operator's file, and many sites prefer an upright page that fits a folder or a notice board. This portrait version suits sites that train in groups, such as distribution centres, factories and equipment dealers, where each operator gets a printed copy after the practical evaluation.
Wording on the template“has completed the classroom training and passed the practical operator evaluation for powered industrial trucks, demonstrating pre-use inspection, load stability, safe travel and pedestrian awareness.”More wording ideas for forklift operator training certificates

Post for the recipient to share
I have finished my forklift operator training and passed the practical evaluation. The course covered the pre-use inspection, load stability and how to stay aware of people on the floor. Thank you to my trainer for the time and the patience. Safe handling is a habit, and I am glad to have my certificate. #ForkliftSafety #WarehouseLife #OperatorTraining

Printing
Printing it well
Print at actual size
Choose “Actual size” or 100% in the print dialog, not “Fit to page”, so the margins and the QR code keep their size.
Use card, not paper
A matte or silk card of 200–300 gsm (about 80–110 lb cover) feels like an award and does not curl in a frame.
Frame it
A4 fits a standard 21 × 29.7 cm frame; US Letter fits an 8.5 × 11 in frame, or an 11 × 14 in frame with a mat. Hang it upright, as designed.
Mind the edges
Most home printers leave a thin white border. For a design that runs to the edge, use a print shop or your printer’s borderless setting.
